<p><b>Weekdays:</b> Monday to Friday</p>
<p><b>Hourly Rate of Pay:</b> $100 - $120</p>
<h3>Position Overview</h3>
<p>Carecor is seeking compassionate Nurse Practitioners (NPs) to deliver virtual and mobile substance‑use care to individuals experiencing homelessness in Toronto. Using a harm‑reduction and client‑centered approach, NPs provide assessment, treatment, and crisis support for clients with complex substance‑use disorders, including opioid, alcohol, and stimulant use disorders.</p>
<h3>Key Responsibilities</h3>
<ul>
<li>Conduct comprehensive assessments, diagnose, and develop client‑centered care plans using a harm‑reduction approach.</li>
<li>Provide accessible virtual/mobile substance‑use care, including evidence‑based treatments and pharmacotherapy (e.g., OAT, anti‑craving medications, withdrawal management).</li>
<li>Address social and structural determinants of health impacting client wellbeing.</li>
<li>Collaborate with physicians, nurses, shelter staff, and community partners to ensure coordinated care and connect clients to longer‑term support.</li>
<li>Maintain accurate documentation using the EMR system and support policy, guideline, and medical directive development.</li>
<li>Contribute to nursing team knowledge through training and support initiatives.</li>
<li>Perform additional nursing duties as assigned by the Nurse Manager.</li>
</ul>
<h3>Job Qualifications</h3>
<ul>
<li>Current registration as an NP (EC) in good standing with the College of Nurses of Ontario; Master’s degree required.</li>
<li>Liability/malpractice insurance, current BLS certification, and a valid vulnerable sector police check.</li>
<li>Minimum 3 years of nursing experience preferred; experience in substance use, primary care, street outreach, or community health is an asset.</li>
<li>Demonstrated expertise in harm‑reduction approaches, mental health, and substance‑use treatments.</li>
<li>Commitment to anti‑oppressive and anti‑racism principles.</li>
<li>Additional training in addictions or controlled substances (e.g., CAMH, OAT) considered an asset.</li>
<li>Strong clinical judgment, communication, organizational, and interpersonal skills; ability to work effectively on an interdisciplinary team.</li>
<li>Proficiency with EMR systems (e.g., OSCAR), Teams, and general computer literacy.</li>
