<p><h3>Overview</h3>
<p><b>Summary Of Duties</b> To provide a full range of social work services to Neurovascular/Stroke patients and their families. Services include psychosocial assessments, counselling, discharge planning, linkage to community resources and participating as a key member of the patient care team.</p>

<h3>Qualifications/Skills</h3>
<ul>
<li>Master’s Degree in Social Work</li>
<li>Member in good standing of the Ontario College of SW and SSW</li>
<li>Minimum 2 years of experience working as a MSW in a Hospital preferred</li>
<li>Commitment to Person Centred Care Best Practices</li>
<li>Excellent communication, organization skills and time management skills</li>
<li>Strong teambuilding skills and conflict resolution skills</li>
<li>Excellent assessment, interviewing and problem solving skills</li>
<li>Knowledge of the health care system and community resources</li>
<li>Superior ability to manage multiple tasks and set priorities in a fast-paced environment</li>
<li>Knowledge of and past clinical experience in Inter Professional Collaboration and Inter Professional Education</li>
<li>Commitment to student education</li>
<li>Interest in exploring Academic Practice opportunities that address Patient and Family Care across the continuum of Health Care</li>
