This position is ideal for therapists who value flexibility, autonomy, and meaningful client relationships within a community-based model. Please note, this position will require <strong>in-person home visits </strong>with vulnerable community members. </p><p></p><p>While we thank all interested applicants, we are only able to contact those who we are inviting to an interview. </p><p></p><p><strong>Key Responsibilities: </strong></p><ul><ul><li>Complete <strong>initial in-person home assessments within 24 hours</strong> of referral, including functional assessments, mobility evaluations, equipment needs, and ADL/IADL analysis </li><li> Provide <strong>follow-up visits</strong> (30-minute or 15-minute sessions) focusing on goal progression, safety, mobility, environmental modifications, and skill building </li><li> Deliver client-centered care aligned with <strong>HCCSS/OH@H expectations</strong>, including timely communication and adherence to care pathways </li><li> Develop and implement <strong>individualized treatment plans</strong> based on client goals, safety considerations, and clinical best practices </li><li> Educate and coach <strong>clients, families, and caregivers</strong> on functional strategies, equipment use, home safety, and risk mitigation </li><li> Coordinate with <strong>interdisciplinary providers</strong> (PTs, PSWs, nurses, case managers) to support seamless care transitions </li><li> Document all assessments, treatment plans, progress, and communications in a <strong>clear, timely, and clinically compliant manner</strong></li><li> Recommend, trial, and prescribe <strong>assistive devices and home modifications</strong>, collaborating with vendors or contractors when needed </li><li>Support in ordering supplies through OH@H vendors </li><li> Monitor client progress and modify care plans as appropriate to support meaningful, measurable outcomes </li><li> Maintain communication with the office regarding caseload availability, scheduling updates, and urgent visit readiness </li><li> Uphold The Care Company’s commitment to <strong>safe, high-quality, ethical, and compassionate community care</strong></li></ul></ul><p><strong>Requirements</strong></p><p><strong>Scheduling: </strong></p><ul><li><strong>Flexible schedule, mainly Monday-Friday, with occasional Saturday/Sunday availability required</strong>, based on client need </li></ul><ul><li>Able to complete an <strong>i</strong><strong>nitial in-person home assessment within 24 hours</strong> of receiving a visit request within geographic area</li></ul><ul><li> Therapists are <strong>assigned geographical zones. </strong>Clients can be located within <strong>Central Region</strong> (may include Richmond Hill, Markham, Newmarket, Simcoe, Keswick)</li><li> Ability to manage scheduling independently and communicate promptly with the office regarding availability </li><li> Travel between client homes required; access to a reliable vehicle is an asset. </li></ul><p></p><p><strong>Qualification and Experience Requirements: </strong></p><p></p><ul><li> Minimum <strong>2 years of experience providing home and community care</strong>, specifically with <strong>HCCSS</strong> and/or <strong>OH@H (Ontario Health at Home)</strong> clients strongly preferred</li><li> Registered and in good standing with the <strong>College of Occupational Therapists of Ontario (COTO)</strong></li><li><strong>ADP authorizers are preferred</strong></li><li> Demonstrated skill in providing functional assessments, equipment recommendations, Equipment and supply ordering through OH@H vendors, safety evaluations, and ADL retraining in the home environment </li><li> Strong understanding of community discharge planning, fall prevention, mobility supports, and caregiver training</li><li>Skilled at electronic documentation </li><li> Experience coordinating with interdisciplinary teams and communicating with case managers </li><li> Ability to work independently in the community setting and produce clear, timely clinical documentation </li><li> Valid driver’s license and access to reliable transportation (or ability to travel efficiently within designated area)</li></ul><p><strong>Benefits</strong></p><p><strong>Compensation: </strong></p><ul><li> $150 per initial assessment, including supply ordering and funding applications.</li><li> $75 per in-person follow-up visit </li><li> $50 per virtual follow-up visit </li></ul><p></p><p><strong>Other Benefits: </strong></p><ul><li>PPE provided</li><li>Mileage Reimbursement</li><li>Visits scheduled to minimize travel</li><li>Educational Training Opportunities</li><li>Employee Recognition Program</li><li>Health and Wellness Program</li><li>Staff & service provider events</li><li>Opportunity for Group Health and Dental Benefits <br><br>#OTCT</li></ul></p></p>Create Your Resume First
