<h3>Job Description</h3><p>Job Description<p><strong>Nurse Practitioner – Community Health Care CHC (Full-Time, Permanent)</strong><br /><strong>Location:</strong> Scarborough, ON<br /><strong>Hours:</strong> 35 hours per week<br /><strong>Compensation:</strong> $51.29–$72.65/hour<br /><strong>Employment Type:</strong> Permanent, Full-Time<br><br><strong>About the Role</strong><br /> We’re seeking a passionate <strong>Primary Health Care Nurse Practitioner</strong> to deliver comprehensive, community-based primary care to unattached children and families. You’ll work collaboratively within an interprofessional model to promote wellness, manage chronic conditions, and provide accessible, high-quality healthcare to those who need it most.<br><br><strong>Key Responsibilities</strong><br /><ul><li>Provide holistic, family-centered primary care services.</li><li>Assess, diagnose, and treat acute and chronic illnesses.</li><li>Develop and implement client-centered treatment plans.</li><li>Provide routine and catch-up immunizations per Ministry of Health schedules.</li><li>Maintain accurate, confidential health records.</li><li>Participate in interprofessional consultations, case conferences, and chart audits.</li><li>Contribute to outreach and health promotion initiatives.</li><li>Support quality improvement and best-practice activities.</li><li>Conduct home visits as required.</li></ul><br /><strong>Qualifications</strong><br /><ul><li>Master’s degree with <strong>Primary Health Care Nurse Practitioner certification</strong>.</li><li>Active registration with the <strong>College of Nurses of Ontario (Extended Class)</strong>.</li><li>3–5 years’ NP experience in a primary or community health setting.</li><li>Current <strong>CPR (HCP)</strong> certification.</li><li>Strong interpersonal, organizational, and communication skills.</li><li>Ability to work both independently and collaboratively in diverse settings.</li><li>Experience with electronic health record systems (e.g., Nightingale on Demand).</li><li>Valid Ontario driver’s license and access to a vehicle preferred.</li></ul><br /><strong>Why Join TAPP Workforce Solutions</strong><br /> At TAPP, we don’t just fill roles — we build careers and communities.
